Medical Director and the Patient’s Physician
Hospice Medical Director provides advice and works collaboratively with the Hospice team and the attending physician to assure that the medical care you receive is closely monitored and meets your ongoing needs.
Nurses
Our hospice nurses offer support 24 hours a day, seven days a week to respond to a crisis or medical emergencies. They make visits as necessary to assess medical needs, and they provide education to the individual and caregivers involved how to best manage a patient’s condition. They also provide expertise in pain and symptom management.
Social Workers
Social Workers assists with financial, legal, and insurance issues, and help find community resources for additional support to families. They also provide counseling and planning to help resolve issues that may occur with a prolonged patient illness.
Chaplains
Chaplains provide spiritual support to patients and families. They connect and work together with your local spiritual leader to help better understand the hospice process. They also assist with funeral arrangements.
Volunteers
Our Volunteers from the community dedicate their time to offer companionship, social, and emotional support, as well as a respite to caregivers.
Bereavement Specialist
We have an extensive grief and bereavement program at Sunnyside Hospice and Palliative Care. We offer grief and loss support – a whole year of bereavement services for loved ones.
Home Health Aides
These caring individuals are some of the few important components of our team. They help with personal care needs, such as feeding or bathing assistance, shaving or nail care, meal preparations, and light housekeeping. They are assigned based on the personal care needs identified in the patient’s plan of care.
